Comments:
The inspector conducted an unannounced monitoring inspection on Monday, November 14, 2022 between 9:00 AM until 12:00 PM. Today?s census included 48 children and 11 staff supervising. During the tour, the inspector observed the children playing with age appropriate materials, eating breakfast, participating in circle time, and interacting with peers and staff. The inspector observed all 7 classrooms, playground, and restrooms. The classrooms are equipped with age and stage appropriate materials. The center's first aid kit and non-medical emergency supplies were inspected and found complete. Liability insurance, health inspection, and drills were all in compliance. A total of 5 child records, and 5 staff records were reviewed. Violations were cited during today?s inspection

Violations:
Standard #: 22.1-289.035-B-4
Description: Based on review of staff record and interview, the center did ensure to obtain a copy of results of an out of state central registry search.
Evidence:
1.Staff #4?s (date of hire: 9/8/2022) file did not contain documentation of an Out of State Central Registry background.
2. The Sworn Disclosure form for Staff #4 disclosed having lived outside Virginia in the past 5 years.

Plan of Correction: HR will follow up on this action.

Standard #: 8VAC20-780-280-B
Description: Based on observation of the physical plant, hazardous substances, such as cleaning materials, were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. A bottle of bleach water was stored on a counter in the preschool classroom.
2. A bottle of bleach water was stored on a counter in the infant 2 classroom.
3. A bottle of bleach water was hanging on a wall above the changing table in the toddler classroom.

Plan of Correction: Remind staff.

Standard #: 8VAC20-780-570-E
Description: Based on observation, the center failed to ensure that all prepared infant formula was labeled with the child?s name and date.
Evidence:
1. In Infant Classroom there were two bottles with prepared formula that were not labeled with a date.

Plan of Correction: Remind staff and parents.

Standard #: 8VAC20-820-120-E-2
Description: Based on observation and interview, the center failed to post the most recent inspection of the facility.
Evidence:
1. The most recent inspection was not posted.
2. Posted inspection was dated 9/8/2021.
