Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A code compliance inspection was initiated on February 1, 2021 and concluded on February 2, 2021. The director was contacted by telephone to initiate the inspection. There were 78 children present and ten staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 12 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on record review the center failed to ensure that central registry findings are obtained within the required time frames.
Evidence:
1 - There are not any background check findings on file for staff 2, 3, 4 or 5.
2 - There is not sworn statement or affirmation on file for staff 3
3 - Staff 1 confirmed in writing that there are not central registry results on file for staff 2, 3, 4, or 5. Nor is there a sworn statement on file for staff 3.

Plan of Correction: Going forward center management will ensure that all background checks are obtained and maintained on file at the center as required.

Description: Based on record review and interview the center failed to ensure each staff has fingerprint background on file as required.
Evidence:
1 - The record for staff 2 and 3 do not contain fingerprint background check results
2 - Staff one stated in writing the records for staff 2 and 3 do not contain fingerprint background check results.

Plan of Correction: Going forward center management will ensure that all background checks are obtained and maintained on file at the center as required.
