Out of state child protective services searches were discussed with the program director.

An unannounced, mandated renewal inspection was conducted today. The inspector arrived at 11:00 AM and departed at 1:45 PM. Twelve children were in care. A sample size of five children's records and five staff records were reviewed. Lunch service and nap time were observed. It was reported that no medication is administered to children in care. Injury records were reviewed.

Standard #: 22VAC40-185-70-A Description: Based upon review of five staff records and staff interview, the facility has not ensured that the record for each staff person includes the name, address, and telephone number of a person to be contacted in an emergency.
Evidence:
1. The records provided for staff 4 and 6 do not include names, addresses and telephone numbers for emergency contact persons.
2. Staff 1 verified that the records for staff 4 and 6 do not iclude emergency contact information.Plan of Correction: The facility responded with the following:
All staff records will include the name, address and telephone number for a person to be contacted in an emergency.
Standard #: 22VAC40-185-270-A Description: Based upon observation and staff interview, the facility has not ensured that areas and equipment inside and outside of the facility are maintained to be safe.
Evidence:
1. There is an one foot square area where the paint is peeling away from the cinder block wall under the window in the toddler room. This area is approximately eight inches from the floor, within reach of the children.
2. There are three areas along the back fence where the ground has eroded along the bottom of the fence, creating gaps that a child could possibly squeeze through allowing access to the water and ravine behind the building.
3. Staff 1 observed the peeling paint in the toddler room and the eroded areas under the fence with the inspector.Plan of Correction: The facility responded with the following:
1. All painted areas will be checked for peeling paint. Peeling areas will be sanded and repainted.
2. The groundskeeper will fill in the eroded areas under the fence and create a barrier to ensure that children cannot get under the fence.
