The Virginia Department of Education (VDOE) assumed oversight of child care licensing and monitoring on July 1, 2021. All associated regulations and codes have transferred over from the Virginia Department of Social Services. The violations listed below reflect the new VDOE codes and standards.

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on 12/09/2021 and concluded on 12/09/2021. The Center Supervisor was contacted by telephone and a virtual inspection was conducted. There were 30 children present, ranging in ages from 5 to 12, with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records were reviewed. 4 Staff records were reviewed for specific components only, as the full staff record is reviewed at the main office at another time. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-80-B-2 Description: Based on a review of documentation, no report was made to the department's representatives within 2 business days of an injury requiring outside medical attention.
Evidence:
On 10/28/21 child #1 hit their head in the gym before school while playing a game (8:20 am). First aid was given, parent notified (8:25 am) and the child went to school. The center was notified at 3:25 pm by the parent that the child had been taken to the ER and was diagnosed with a concussion after going to the school clinic complaining of a headache and throwing up.
As of 12/9/2021 no report was made to the department's representative.Plan of Correction: Moving forward, I will follow up with supervisors to make sure any serious injuries are reported.
Standard #: 8VAC20-780-245-J-1-b Description: Based on a review of available documentation, Medication Administration Training (MAT) certification was not renewed within the allowable 3 year time frame.
Evidence:
MAT certification for Staff A (only staff certified) expired 12/4/21. The recertification class was not taken until 12/9/21. There are emergency medications on site.Plan of Correction: I will make sure I contact the people responsible for training to ensure someone is always current in MAT certification.
I passed the class on 12/9/21 and have the receipt from that class.
