Technical Assistance:
Follow-up to requested child protective services central registry searches was discussed.

Care and record keeping for children with diagnosed food allergies/food sensitivities/dietary
preferences was discussed.

Comments:
A monitoring inspection was initiated on 01/31/2022 and concluded on 02/02/2022. There were 48 children present with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 children's records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-40-M
Description: Based upon staff interview, the facility has not ensured that a current written list of all children's allergies, sensitivities, and dietary restrictions is maintained and documented in the the allergy plan that is dated and kept confidential in each room or area where children are present.
Evidence:
Staff 4 stated that an allergy notice is kept in the classroom where a child with an allergy is in care. The list is not dated and is not kept in each room where children are preset.

Plan of Correction: The facility responded with the following:
A format was implemented in which the registry of all children who present some type of allergy is found, each teacher knows in her respective room which child has the allergy, and a distinctive note is found on each hook (child name).

Standard #: 8VAC20-780-60-A
Description: Based upon review of children's records and staff interview, the facility has not ensured each child's record includes the names, addresses and telephone numbers of two persons to be contacted in an emergency when a parent cannot be reached.
Evidence:
The record provided for child 4 did not include the names, addresses and telephone numbers for two emergency contact persons.

Plan of Correction: The facility responded with the following:
The same day in the afternoon hours, his parent or guardian correctly filled out the respective registration sheet.

Standard #: 8VAC20-780-280-B
Description: Based upon observation and staff interview, the facility has not ensured that hazardous substances are kept in locked place using a safe locking method that prevents access by children.
Evidence:
1. There was a container of Microban and a spray air freshener (with warning labels) on an open high shelf in the girls' bathroom.
2. There was a container of Aquafor in a low child's cubby in the girls' bathroom.
3. There was a container of Lysol spray on the floor in the Pre-K classroom.
4. The cabinet under the sink in the open kitchen did not have the child proof latch connected. The cabinet contained various cleaning products.
5. Staff 1 acknowledged the unlocked hazardous substances during the facility tour.

Plan of Correction: The facility responded with the following:
The material was placed in a safe place during the inspection.
Planned to purchase raised all cabinets with padlocks.

Standard #: 8VAC20-780-340-F
Description: Based upon observation and staff interview, the facility has not ensured that children under 10 years of age are always within actual sight and sound supervision of staff.
Evidence:
1. A four year old girl was observed in the girls' bathroom. There was no staff person within sight and sound supervision of the girl.
2. Staff 1 verified that no staff person was within sigh and sound supervision of the the bathroom.

Plan of Correction: The facility responded with the following:
On the same day in the afternoon hours, a feedback meeting was held with all staff and the permanent order was made clear to carry out accompaniment and supervision of every child who uses the bathroom, except children who go to elementary school.

Standard #: 8VAC20-780-560-F-4
Description: Based upon review of the posted menu, the facility has not ensured that the menu is dated.
Evidence:
The menu posted in the facility foyer was not dated.

Plan of Correction: The facility responded with the following:
It was corrected and posted the same day during the inspection.
