Comments:
The inspector conducted an unannounced monitoring inspection on May 24, 2023, with center director. The inspection began at approximately 9:30 AM and concluded at 12:00 PM. There were 49, children present, ranging in ages from 4 months to 5 years, with 7 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-130-A
Description: Based on a review of children's records, the provider did not ensure that each child's record contained documentation of immunizations prior to attending the child day center.
Evidence:
1. Child #1 record contained documentation of immunizations 2/23/2023. Child #1?s date of enrollment was 2/20/2023.
2. Child #4?s record contained documentation of immunizations 3/1/2023. Child # 4?s date of enrollment was 3/1/2023.

Plan of Correction: Going forward, I will make sure immunizations received by start date.

Standard #: 8VAC20-780-270-A
Description: Based on review of the facility and outdoor play areas the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.
Evidence:
1. The steering wheel the red metal car located on the preschool playground was broken and contained sharp edges.
2. There were three nails protruding from the wood on the red playhouse located on the preschool playground.

Plan of Correction: Will review with staff.

Standard #: 8VAC20-780-500-C
Description: Based on observations the center failed to ensure that staff empty, clean, and sanitize toilet chairs (potty chairs) after each use.
Evidence:
1. In the 2?s classroom, two potty chairs contained a yellow substance. The potty chairs were not emptied promptly, cleaned and sanitized after each use.

Plan of Correction: Playground repairs scheduled PM 5/24/2023.
