A monitoring inspection was conducted on 09/27/2022 between the hours of approximately 3:30 pm and 6:15 pm. There were 19 children present, ranging in ages from 5 years to 10 years, with 3 staff supervising. Children were observed while eating snack, transitioning to their classrooms, doing homework and engaging in toy and free play. Staff was observed actively supervising.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-160-A Description: Based on record review, the center did not ensure that documentation of negative tuberculosis (Tb) screening for each employee met all requirements.
Evidence:
1) The documentation shall have been completed within the last 30 calendar days of the date of employment. The Tb screening form in Staff #1's (start date 09/07/2021) record was dated 1/29/2021.Plan of Correction: Staff #1 will have a Tb screening done to comply with the requirements.
Standard #: 8VAC20-780-160-C Description: Based on record review, the center did not ensure that employees obtain and submit the results of a follow-up tuberculosis (Tb) screening at least every two years.
Evidence:
1) A current Tb screening was due for Staff #2. The Tb screening form in the record for Staff #2 was dated 09/17/2020.Plan of Correction: Corrected on 10/04/2022.
Standard #: 8VAC20-780-280-B Description: Based on observation, hazardous substances were not kept in a locked place that prevents access by children.
Evidence:
1) While the children were eating snack in the cafeteria, two bottles of Spic and Span cleaner were observed on a table to the side of the cafeteria.Plan of Correction: All hazardous substances will be kept under lock and key starting today.
Standard #: 8VAC20-780-560-F Description: Based on observation and interview, the center did not met all requirements when providing snacks.
Evidence:
1.The program did not follow the most recent, age-appropriate nutritional requirements of the Child and Adult Care Food Program of the United States Department of Agriculture (USDA) which states that snacks shall include two food components. On the date of inspections, the snack listed on the menu was Cheez-it crackers only. When asked for the second component, the staff person interviewed stated that children were offered water. Water is not considered a food component.Plan of Correction: We will have the two food components available starting today at snack time.
