Comments:
A renewal inspection was initiated on 8/31/2023 and concluded on 8/31/2023. There were nine children present with two providers supervising. The point total was 18. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and three staff records were reviewed. The inspection started at 9:50 am and concluded at 12:25 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and date to be corrected? for each violation cited on the violation notice and return it to me within five (5) business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring gain; and 3) person or person(s) responsible for implementation.

Description: Based on observation the provider did not ensure that medication, except for those prescriptions designated otherwise by a written physician's order, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children.
Located on the wooden rail as you enter the child care room was a bottle of soothing tablets, not locked.

Description: Based on record reviews the provider failed to ensure that annually a review with the parent the emergency contact information to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.

The newer wooden climber has a climbing wall that has a rope attached that the children use to climb the wall.

Description: Based on observation the provider failed to ensure that a caregiver does not exceed 16 points by using the point system to determine if an additional caregiver is needed.
Evidence:
Upon arrival to the family day home staff #1 was alone with nine children totaling 18 points. Staff #1 stated that staff #2 had gone out and she had texted staff #2 to come back. Staff #2 returned to the family day home in approximately 10 minutes.
