Description: Based on observation, the provider did not ensure that documentation be maintained of emergency evacuation and shelter-in place drills that includes all required documentation.
Evidence:
There documentation on the providers emergency drill log did not include the following:
1. The method used for notification of the drill,
2. The number of caregivers participating,
3. Any special conditions simulated,
4. Problems encountered, if any,
5. Weather conditions for evacuation drills.

Description: Based on observation, the provider did not ensure that when more than one infant is in care, bottles are labeled with the child?s full name and the date.
Evidence:
There were two bottle fed infants in care. Staff 2 was feeding an infant a bottle during the inspection. The bottle was not labeled with the child?s full name and the date.

Description: Based on record review, the provider did not ensure that children?s records contain all the required information.
Evidence:
1.The records reviewed for child 1, 2, and 3 did not include the policy numbers of their medical insurance coverage.
2.The provider confirmed that the three children?s records had no medical policy numbers included in the space provided on the enrollment form.
