Comments:
A subsidy health and safety (SHSI) inspection was completed on 3/20/2023. A Voluntary Registered Monitoring inspection was also conducted on this date.
Violations were noted and are identified on the violation notice.

Violations:
Standard #: 8VAC20-790-250-F
Description: Based on a review of records and interview, the provider did not obtain the department's health and safety training update course.

Evidence:
The provider's training documentation was reviewed.
Documentation that the provider had completed the department's annual health and safety training update course was not in the record.
The provider stated that she had not completed the department's annual health and safety training update course.

Plan of Correction: The Provider will complete the department's health and safety training update course by the end of the month and successively each year. The certificate of completion will be maintained in the provider's record.

Description: Based on observation and interview, the provider did not ensure that potentially poisonous substances, materials, and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates qre stored in areas inaccessible to children.
Evidence:
The following potentially poisonous substances were observed in an unsecured cabinet under the bathroom sink in the restroom that children use while in care:
Bleach, disinfectant cleaner, non abrasive scrubbing detergent, hand sanitizer, liquid drain cleaner, carpet cleaning spray.
The following potentially poisonous substances were observed on an open shelf of a cart in the kitchen, just off of the main living area where children are cared for:
Hand sanitizer, disinfectant spray, deodorizer spray.
All of the listed items contained warnings on the label to include "keep out of reach of children" and additional hazard warnings.
The provider acknowledged that these items were located in areas that were accessible to children.

Plan of Correction: The provider stated that she carefully observes the children to ensure that they do not access these items while they are in care - but that she would relocate these and any other hazardous substances to an area not physically accessible to the children in care.
