An unannounced monitoring inspection was conducted on 3/29/2023 from 10:30am to 12:20pm. There were 42 (4 months to 5 years) directly supervised by 7 staff. The physical plant, 5 staff records, 5 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, eating lunch, and preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 8VAC20-780-160-C Description: Based on observation, the facility did not ensure that at least every two years from the initial screening, staff shall obtain and submit results of a follow-up tuberculosis screening.
Evidence: Reviewed 5 staff files and found that Staff 2's last tb screening was on 11/27/2020Plan of Correction: Staff 2 received an updated negative TB screening on March 5, 2023.
Standard #: 8VAC20-780-270-A Description: Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, are maintained a clean, safe and operable condition.
Evidence:
1) observed on the toddler playground, weed matting coming up through the mulch, causing a tripping hazard.
2) observed a small hole in the hall in the Twos classroom
3) observed nails protruding from the fence on the preschool playground.Plan of Correction: Weed matting was trimmed following inspection. Hole in the wall of the 2's classroom was covered. We have a company scheduled to come repair drywall and paint the entire facility. Protruding nails have been removed. We are in the process of contracting a company to replace all fencing.
Standard #: 8VAC20-780-350-B-1 Description: Based on observation, the facility did not ensure that ratios of 1:4 for children birth to 16 months of age.
Evidence: observed that the infant room had 9 children with 2 staff.Plan of Correction: A third teacher for the Infant room arrived at 11:30 a.m. putting the classroom in ratio.
Standard #: 8VAC20-780-350-B-2 Description: Based on observation, the facility did not ensure that the ratio for children 16 months to 24 months as 1 staff to 5 children.
Evidence: observed in the toddler classroom, 7 children and 1 staff.Plan of Correction: Second Teacher in the toddler classroom resigned with no notice.

Standard #: 8VAC20-780-350-B-4 Description: Based on observation, the facility did not ensure that ratios for 3 year olds and up was 1:10.
Evidence:
1) observed in the Three year old classroom, 11 children and 1 staff
2) observed in the 4s and 5s classroom, 24 children and 1 staffPlan of Correction: Second Teacher in the 4's and 5's classroom resigned with no notice to handle a family emergency out of the country.
