This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A code compliance inspection was initiated on 02/17/2021 and concluded on 02/17/2021. The director was contacted by telephone to initiate the inspection. There were 33 children present and 4 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 4 staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable code sections and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Standard #: 22VAC40-191-40-D-2 Description: Based on staff record review, the center did not ensure four of four staff records had documentation of central registry results within 30 days of employment and / or documentation of a sworn statement or affirmation prior to employment.
Evidence:
1. The record of staff #1 (start date: 6/4/18) had documentation of central registry results dated 7/30/18 and documentation of a completed sworn statement of affirmation dated 2/17/21.
2. The record of staff #2 (start date: 8/20/18) did not have documentation of central registry results and documentation of a completed sworn statement of affirmation dated 11/28/18.
3. The record of staff #3 (start date: 8/27/18) had documentation of a completed sworn statement of affirmation dated 11/28/18..
4. The record of staff #4 (start date: 8/30/10) did not have documentation of central registry results.Plan of Correction: We will make sure fingerprint results and sworn statement forms will be done before employment and central registry results received within 30 days.
Standard #: 63.2(17)-1716-B-3 Description: Based on record review and an interview, the center did not ensure to have at least one person on staff that has been trained to conduct daily simple health screenings of children.
Evidence:
During the inspection of four staff records and interview with the center representative, it was discovered there were no staff that received training in daily health screenings. Religious exempt requirements stipulate that a staff should be able to conduct a daily simple health screening and exclusion of sick children by a person trained to perform such screenings.Plan of Correction: We have paid for the Daily Health Screening Training and as soon as we get the code to complete the training we will complete it.

Evidence:
1. The record of staff #1 (start date: 6/4/18) had documentation of fingerprint based national criminal history search results dated 2/8/21.
2. The record of staff #2 (start date: 8/20/18) had documentation of fingerprint based national criminal history search results dated 12/3/18.
3.
