This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was completed on June 11, 2021. The provider was contacted by telephone (video call) to initiate the inspection. There were 37 children present and in the care of eight staff members. The inspector was able to view all required documentation. A virtual tour of the children in care was conducted. Three staff records were entirely reviewed and the background checks were reviewed for nine staff members. Three children's records were reviewed and the medication and medication paperwork were reviewed for one child. Information gathered during the inspection determined non-compliances with applicable standards or law and the violations were documented on the violation notice issued to the facility.

Standard #: 22VAC40-185-60-A Description: Based on review of three children's records and verification from the director, the three records did not contain all of the required enrollment information.
Evidence:
1. The records for Child #1, Child #2 and Child #3 did not contain the name, address and phone number of a second person to contact in case of an emergency and the child's parent cannot be reached.
2. The records for Child #1, Child #2 and Child #3 did not contain the three required signed written agreements between the parent and the center. Date of enrollment for all three children was 06/07/2021.
3. The director verified that the parent agreements and the second emergency contact had not been obtained from the parents.Plan of Correction: We have adjusted the online registration to include 2 emergency contacts and parent agreements. For Monday's session we will use a hard copy to have parents sign the parent agreements and add a second emergency contact.

2. The director confirmed there was nothing on the medication to show it had been prescribed to Child A.Plan of Correction: The parent will be informed at pick-up time that an unexpired emergency medication must be provided to us in the prescription box with the prescription label attached before the child returns to camp.
