Comments:
An unannounced renewal inspection was conducted on 10/24/2023. There were 73 children, ages 2 years - 5 years, 11 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies and nutrition. The children were observed during teacher directed activities, self-directed activities, outdoor time, mealtime and nap time. A total of 8 children?s records, 8 staff records, 1 agent/applicant record, and 7 medications were reviewed. The inspector discussed the following with the provider: snack and over the counter skin products. The inspector arrived for the inspection at approximately 10:00 am and departed approximately 1:10 pm.

The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

After review with the Licensing Administrator, the inspection documents were updated on 12/18/2023 to correct an administrative error.

Description: REPEAT VIOLATION
Based on observation, the center failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

Evidence:
1. The resilient surfacing (mulch) under the climbing equipment with blue slide measured approximate one inch. The ground was visible at the base of the slide.
2. The resilient surfacing (mulch) under the 2 pieces of climbing equipment with yellow slides measured approximate one inch. The ground was visible at the base of the slide.
3. The resilient surfacing (mulch) under the dome climbing equipment measured approximate one inch. The ground was visible in the center of the dome.

Plan of Correction: Provider will obtain additional mulch.
