Comments:
An unannounced monitoring inspection was conducted on June 8, 2023 from approximately 1:30 pm to 2:30 pm. There were 20 children in attendance and a total of 4 staff present. 2 staff records and 2 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed taking a nap and reading books while the licensing inspector was on-site.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 06/14/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on records review, and observation, the center failed to ensure employees undergo background checks every five years. Evidence:
1) The record for staff #1, date of employment, 06/01/2017 contained a criminal background check dated 05/17/18.
2) The record for staff #2, date of employment, 03/01/2016, contained a criminal background check dated 05/29/2018.

Standard #: 8VAC20-780-40-M
Description: Based on observation, and interview, the center failed to maintain, in a way that is accessible to all staff who work with children, a current written list of all children?s allergies, sensitivities, and dietary restriction.
Evidence:
1) Classroom #1 contained an allergy list dated 2021-2022.
2) When asked to see the allergy list in classroom #2 staff #3 stated that there was not one.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition. Evidence: There were several broken, cracked, and deteriorating wood slats on the fence surrounding the playground.

Description: Based on observation, and interview, the center failed to ensure that restrooms used by children shall be equipped with soap, toilet paper, and disposable towels. Evidence: The restroom located in classroom #2 did not have soap, toilet paper, or paper towels. When interviewed staff #3 stated that was the restroom the children use.

Standard #: 8VAC20-780-330-B
Description: Based on observation, the center failed to ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall encompass sufficient area to include the child?s trajectory in the event of a fall.
Evidence:
1) There was less than an inch of mulch surrounding the fall zones of the slide. When mulch is used as the resilient surfacing there must be at least 6 inches depth in the fall zones.
2) The mulch was less than 6 feet in length in the area surrounding the fall zones.
