63.2(17) License & Registration Procedures

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on October 7, 2020 and concluded on October 9, 2020. The director was contacted by telephone to initiate the inspection. There were thirty-two children present and three staff. The inspector emailed the director a list of items required to complete the inspection. The inspector reviewed three children?s records and three staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review, facility did not ensure three of three children's records have documentation of required information.

Evidence: The following was found during review of children's records:
1. The records for child #1 (start date September 8, 2020) and child #3 (start date September 8, 2020) did not have documentation of parent's employment information.
2. The record for child #2 (start date September 8, 2020) did not have mother's employment phone number and a second emergency contact other than a parent.

Plan of Correction: I plan to carefully review registration forms. Also plan to have parents provide employment phone numbers.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on review, the facility did not ensure one of three staff have an Office of Background Investigations (OBI) fingerprinting determination letter prior to a staff beginning employment.

Evidence: The record for staff #2 (hire date June 30, 2020) had a fingerprinting determination letter dated July 23, 2020. Documentation stated orientation training was completed July 20, 2020.

Plan of Correction: Plan to complete OBI before onboarding.
