An unannounced renewal inspection was conducted today from 10:30am-11:15am. There were 5 children directly supervised by 2 staff. The home, 2 staff records, 5 children?s records, 3 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely.

Based on record review, children's records did not contain all required information.
Evidence:
1. Child 1's most recent annual update was completed 12/13/21.
2. Child 4's most recent annual update was completed 6/1/2022 and did not contain the name of the child's health insurance carrier or policy number.
3. Child 5's most recent annual update was completed 6/13/2022.Plan of Correction: I will have the parent's review and or provide all relevant information.

Based on record review, parent's were not provided all required information in writing.
Evidence:
1. Child 2's record did not contain documentation that parent's have been informed of the provider's medication policy.
2. Child 3 and 4s record did not contain documentation that the parent's have been informed of the provider's termination policy.
3. Child 5's record did not contain documentation that the parents have been informed of the provider's policies.Plan of Correction: I will have the parent's sign off that they have been informed of all required policies.
