Technical Assistance:
Provided consultation on standard: 160 Attributes for Household Members - Individuals 14 years of age and older who reside in the family day home shall meet the requirements specified in 22VAC40-191 Backbround Checks for Child Welfare Agencies;

Provided consultation on: General Guidance for Reopening Child Care Programs During Phase II - distributed to all providers on 6/2/2020.
- Follow CDC's "Guidance for Schools and Child Care Programs"
- Implement enhanced social distancing measures
Provided consultation on: Infection Control and Sanitation Practices During Phase II
- Implement robust policies and procedures for handwashing and cleaning and sanitizing frequently touched surfaces
- Clean (with soap and water) and disinfect surfaces with EPA-approved disinfectants
- Avoid using items that are not easily cleaned, sanitized or disinfected

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 6/22/20 and concluded on 6/23/20. The provider was contacted by telephone to initiate the inspection. There were six children present with the provider supervising. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed two children's records and two provider/household member records to ensure documentation was complete. The information gathered during the inspection determined non-compliance with the standards that are documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of two children's records, the provider did not obtain documentation that each child has received a physical examination by or under the direction of a physician prior to the child's first attendance or within one month after attendance. Evidence: the records for child #1 (date of first attendance: 1/14/20) and child #2 (date of first attendance: 5/6/19) did not have documentation of a physical examination.

Plan of Correction: Will ask the parents for the physical

Standard #: 22VAC40-111-70-A
Description: Based on review of two children's records, the provider failed to have documentation that all policies were provided to parents in writing before the first day of attendance. Evidence: The record for child #2 did not have signed documentation that parents had been given VDSS model form "Liability Insurance Declaration."

Plan of Correction: Will ask the parents to sign the form

Standard #: 22VAC40-111-210-A
Description: Based on review of documentation, and an interview with the provider, it was determined that the provider did not complete 16 hours of annual training. Evidence: the provider stated that she had completed 10 of the required 16 hours of training.
