Violations:
Standard #: 8VAC20-800-960-C
Description: Based on observation, the provider did not ensure that a high chair, infant carrier or feeding table with safety waist and crotch straps fastened were used for children under 12 months of age who are not held while being fed.

Evidence: The provider fed Child #7 (DOB 4/13/22) a bottle while they were laying in a bouncy seat. Child #7 was not strapped in during the feeding.

Plan of Correction: We always used belts for babies in the high chair. I will make sure for next time.

Standard #: 8VAC20-800-960-E
Description: Based on observation, the provider did not ensure that bottles be refrigerated and labeled with the child?s full name and the date, if more than one infant is in care.

Evidence: The bottles for Child #7 were not dated as required.

Plan of Correction: I asked the parents to send the bottles with labels.

Standard #: 8VAC20-800-170-A
Description: Based on review of 2 Caregiver records, the Provider did not obtain a report documenting the absence of tuberculosis for each caregiver.

Evidence: The record for Caregiver #2 did not contain a report documenting the absence of Tuberculosis.

Plan of Correction: I asked my assistant to bring the X-ray report from her doctor separately.

Standard #: 8VAC20-800-240-A
Description: Based on observation, the Provider did not ensure that areas and furnishings of the family day home, inside and outside were maintained in a clean, safe and operable condition.

Evidence:
1)The seat of the high chair was torn in a number of places and the non-absorbent stuffing inside of the high chair seat was not waterproof or able to be sanitized.
2)There was a broken component on the play equipment outdoors that was hanging from the structure and had a sharp fastener exposed that could snag a child?s skin.

Plan of Correction: I changed the seat cover.
