Had a discussion with the provider regarding discipline techniques. If time out is going to be used as a form of discipline, the provider will need to update their Information for Parents to reflect the changes.

An unannounced monitoring inspection was conducted today between 10:40am and 1:10pm. There were 9 enrolled present children along with two of the provider's children with the provider and 1 assistant for a total of 29 points within the supervision guidelines. The children were observed playing freely, dancing to music with bean bags, cleaning up, washing their hands and eating lunch (pepperoni pizza). There is an ample amount of toys and supplies available to the children. A complete inspection of the physical plant, children and staff records, fire drill log, menu and attendance were observed during this inspection. Thank you for your cooperation. Standard #: 22VAC40-111-830-A Description: Based on record review and interview, emergency evacuation procedures were not practiced monthly.
Evidence: An evacuation drill is partially documented for the month of November and there is no drill documented for December. The provider stated that they can't recall if they practiced a drill in November and they must have forgotten to practice a drill in December.Plan of Correction: I'll start doing monthly reminders.
Standard #: 22VAC40-111-880 Description: Based on observation, meals and snacks provided by the family day home did not follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
Evidence: Today the children were served pepperoni pizza and margarita pizza with water. The children were not offered a fruit, a full serving of vegetables or milk.Plan of Correction: We usually have a fruit and vegetable with lunch. The children don't milk. Usually I would've cooked a lot more.
Standard #: 22VAC40-111-920 Description: Based on observation and interview, when meals were provided by the family day home, a weekly menu was not posted.
Evidence: The children were observed being served lunch. There is no food menu posted. The provider stated that she is currently working on completing the menu.Plan of Correction: I'll get it up today.
Standard #: 22VAC40-111-120-B Description: Based on record review, assistant providers records did not contain all required information.
Evidence: Caregiver #2 file did not contain the caregivers verification of age, job title, date of employment and name and adresss of a person to be contacted in case of an emergency.Plan of Correction: I'll get it done.
Standard #: 22VAC40-111-180-A Description: Based on record review and interview, the provider did not obtain subsequent tuberculosis (TB) screenings for each caregiver and adult household member every two years.
Evidence:
1. The provider's most recent documented TB screening is dated 1/3/2018.
2. Household member #1's most recent documented TB screening is dated 10/3/2019.Plan of Correction: (Household member #1) will go tomorrow and I'll go sometime over the weekend.
Standard #: 22VAC40-111-280 Description: Based on observation, potentially poisonous substances were not stored inaccessible to children.
Evidence:
1. There are multiple aerosol air cleaners, bleach cleaners and a multipurpose cleaner stored under the sink in the children's bathroom. The cabinet is not locked and children have access to the bathroom.
2. There was a container of sanitizer wipes sitting on a table near the children's tables during the inspection. The wipes were at the children's level and accessible.Plan of Correction: I'll have to order new locks.
