An unannounced monitoring inspection was conducted today from 9:00am-10:00am. There were 4 children directly supervised by 1 caregiver. The home, 2 household member records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22.1-289.036-B-1 Description: Based on review of household member (HHM), when a HHM turned 18 years of age a sworn statement of affirmation was not signed.
Evidence
HHM #1 turned 18 and has not submitted a signed sworn statement.Plan of Correction: My child is away at school, she will be home this weekend and I will have her sign the sworn statement.
Standard #: 8VAC20-800-780-B Description: Based on review of children's records, the provider did not annually review the child's record with the parent to ensure all information is updated and correct.
Evidence
The record for child #1 and #2 was not reviewed annually with the parent. The last update for child #1 was 11/7/22. The last update for child #2 was 8/22/22Plan of Correction: I will have the parents review and sign the record and make any updates.
Standard #: 8VAC20-800-170-A Description: Based on review of household member (HHM) records, each HHM did not obtain a report of tuberculosis screening when turning 18 years of age.
Evidence
HHM #1 turned 18 and has not obtained a report of tuberculosis screening.Plan of Correction: My child is away at school, when comes home this weekend I will get a copy of her TB screening.
