Consultation was given on becoming a subsidy vendor and DOE?s QB5 quality program. Subsidy information can be found on childcareva.com under the Provider link. An attachment was sent to the provider on QB5 by email. In addition, discussed new over the counter forms for summer and ensuring expiration dates are current.

A monitoring inspection was initiated on 5/5/2022 and concluded on 5/5/2022. There were a total of 21 children with 8 staff members present. Four classrooms were in operation with ratios maintained by direct care staff. Infant care was observed including diapering and hand washing. Children were observed engaged in group activities and outdoor play time. The inspector reviewed compliance in the areas of administration, physical plant, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-60-A Description: Based on file review, there was no annual documentation to verify that children?s records contained up-to-date information.
Evidence: Child file #1 had not been updated for over a year. (10/5/2016)Plan of Correction: Center will document that all children?s files have been reviewed for changes in information.
Standard #: 8VAC20-780-270-A Description: 1.Based on observation, equipment was not maintained in a safe condition.
Evidence: Metal gate dividing playgrounds was cracked and could snag skin.
2.Based on observation, an area of the center had peeling paint.
Evidence: The wall in the J-K classroom between 2 windows had a spot of peeling paint.Plan of Correction: 1. A work order has been placed to maintenance. Center will have staff block access to the gate until it is repaired.
2. Center has contacted maintenance and the wall will be repaired tomorrow.
Standard #: 8VAC20-780-440-L Description: Based on observation, a prohibited item was in an occupied crib for a child under 2 years of age.
Evidence: A crib contained a soft toy at the center.Plan of Correction: Staff removed the toy during the inspection and will not allow toys in cribs.
Standard #: 8VAC20-780-560-F Description: Based on menu documentation, the center was not following the most recent nutritional requirements of USDA when providing snacks.
Evidence: The menu listed a snack containing only one food group.Plan of Correction: Center will update menu to include correct number of food groups for all snacks.
