A monitoring inspection was initiated on 04/26/2023. The inspector was on site from
1:30pm to 2:55pm. There were a total of 33 children observed with 3 staff member and 1 administrative staff member that arrived after the inspection began. The children were observed napping and eating. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed. Information gathered
during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1)
steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22.1-289.035-A Description: Based on a review of staff records, employees did not undergo a background check in
accordance with subsection B prior to employment or beginning to serve as a volunteer and every five years thereafter.
Evidence:
Staff #3 (date of hire 8/23/2016) had a central registry check dated 4/5/2018.Plan of Correction: Central Registry check was completed on 5/3/2023. Moving forward background checks will be monitored via calendar and submitted prior to expiration.
Standard #: 8VAC20-780-60-A Description: Based on a review of child records, the CDC did not ensure that all required information for each child enrolled was obtained.
Evidence:
Child #1 (date of enrollment 10/14/2021) and child #3 (date of enrollment 10/4/2018) did not have work information provided for the parents.Plan of Correction: Parents were contacted & do not have work addresses as they work from home. Records have been updated to reflect parents have address for their work location.
Standard #: 8VAC20-780-70 Description: Based on a review of staff records, the CDC did not ensure all of the information required for staff records was available.
Evidence:
Staff #2 (date of hire 9/28/2021) only had oneof the two required references on file during the
inspection.
Staff #1 (date of hire 10/26/2021) did not have references on file.Plan of Correction: References for both staff members have been obtained & documented.
Standard #: 8VAC20-780-240-A Description: Based on a review of staff records, the CDC did not ensure that the Virginia Department of Education-sponsored orientation course was completed within 90 calendar days of employment.
Evidence:
Staff #1 (date of hire 10/25/2021) did not have the orientation course completed.Plan of Correction: Staff is currently in the process of completing required pre-service training. All hires will be required to complete training prior to beginning employment.
Standard #: 8VAC20-780-350-B-2 Description: Based on observation, the CDC did not ensure ratios were maintained.
Evidence:
19 children were observed with 1 staff member in the 3 year old classroom during nap time. The ages of children in the classroom were 18 months (date of birth 10/14/2021) to 4 years old (4/9/2019).Plan of Correction: Families of part day children were contacted to make sure children are picked up on time so ratio can be maintained, at all times.
Standard #: 8VAC20-780-350-K Description: Based on observation, the CDC did not ensure an additional staff member was available and on-site to offer immediate assistance. The staff required by rest period ratios shall be able to summon the additional staff member without leaving the room or area of the sleeping or resting children.
Evidence:
Upon arrival at the CDC, there was no one available to answer the door. A phone call to the center confirmed that the Director was not on site and everyone in the building was in charge of children and needed in ratio. When the Director arrived approximatively 10 minutes later the CDC had 3 classrooms open with 1 staff member in each of those classrooms.Plan of Correction: Staff hours have been adjusted to ensure there is always a free staff on site to offer additional assistance to staff without them having to leave their classrooms.
Standard #: 8VAC20-780-540-E Description: Based on emergency supply review and Director interview, the CDC did not ensure all nonmedical emergency supplies were available.
Evidence:
A battery operated radio was not available. The Director confirmed they did not have a battery operated radio at the center.Plan of Correction: Battery operated radios are located in each classroom & have been on site since facility opened in 2016. Batteries are also located in center office at all times.
Standard #: 8VAC20-780-550-D Description: Based on a review of documentation the CDC did not implement monthly practice evacuation
drills.
Evidence: The last documented evacuation drill was 2/22/2023.Plan of Correction: Fire drills will be monitored via calendar to make sure they are completed monthly.
Standard #: 8VAC20-780-550-E Description: Based on a review of documentation, shelter in place procedures were not practiced a
minimum of twice per year.
Evidence:
There was only one shelter in place drill documented for 2022-dated 12/28/2022.Plan of Correction: Shelter in place practices will be monitored via calendar to make sure they are completed twice a year.
Standard #: 8VAC20-780-570-E Description: Based on observation, the CDC did not ensure the bottle was dated.
Evidence:
1 out of 2 sets of bottles were not dated.Plan of Correction: Staff have been retrained to make sure all bottles/food items are dated.
