Technical Assistance:
Discussion on the gate located on the upper level of the home. The provider secured the gate from moving using tire threads on both sides of the iron guardrails.

Comments:
An unannounced renewal inspection was conducted today from approximately 9:40 am and 12:45 pm. There were 8 children (22 points) in care of the provider and two assistants. A walk-through of the entire home was completed.

4 children?s and 3 staff records, household member record, caregiver training, children?s nutrition, supplies, emergency procedures and drills were inspected. The children were observed playing freely and with toys on the floor. Diapering changing and hand washing.
There were age appropriate materials for all children in care. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-790
Description: Based on observation, the provider did not post the responsible person for emergency backup care.
Evidence: The provider did not have posted the responsible person for emergency backup care.

Plan of Correction: I will make a copy and post.

Standard #: 22VAC40-111-240-A
Description: Based on observation and staff interview, areas and furnishings of the family day home, inside and outside, shall be maintained in a clean, safe, and operable condition.
Evidence: There was a visible crack in the wood on the baseboard, located by the left side of the steps entering in to the lower level. The provider stated the wood was in the process of being repaired.

Plan of Correction: In the process of being repaired.

Standard #: 22VAC40-111-430-B
Description: Based on observation, the provider was using a gate as a protective barrier at stair openings.
Evidence:
1. The provider was using an accordion gate to block the stairs on the lower level where children have access by the stair opening.
2. The gate was not mounted.

Plan of Correction: I will get a new gate to mount.

Standard #: 22VAC40-111-440-A
Description: Based on observation and staff interview, the provide did not ensure did not have access to deck that do not have protective barriers or guardrails.
Evidence: The gate located by the deck was mounted; however, the gate was not able to lock to secure that children did not have access to the deck.

Plan of Correction: Will work on securing the gate and will send a picture to the inspector.

Standard #: 22VAC40-111-680-B
Description: Based upon observations and staff interview, the provider did not ensure hands were washed with liquid soap and warm running water after diaper changing.
Evidence: The provider changed Child #3's diaper. The child went to play with toys as the provider washed their hands. The child continued to play with toys until it was bought to the provider's attention that children hands have to be washed with liquid soap and water after diaper changing. The provider washed the child's hand.

Plan of Correction: Provider will continue to take child to wash hands after diaper changing.

Standard #: 22VAC40-111-690-D
Description: Based on observation and staff interview, the provider did not ensure the diapering surface was nonabsorbent, cleaned and sanitized.
Evidence: The provider changed Child #3's diaper. The provider stated she places a diapering linear on the diaper surface The diapering surface was not cleaned and sanitized after each use.

Plan of Correction: I will purchase a new diaper surface and will clean and sanitize after each change.
