Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy health and safety inspection was initiated on May 7, 2021 and concluded on May 11, 2021. The Director was contacted by telephone to initiate the inspection. There were 15 children present and 3 staff on March 6, 2021. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on staff interviews and record review, the vendor did not ensure staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence: The record for staff #1, did not include a TB screening meeting the requirement as it was dated 12-18-20219 and the date of hire was 11-9-2020. The TB screening available for review was conducted more than 30 calendar days prior to hire.

Plan of Correction: Staff will have TB completed. N more than 30 days of for future employment.

Standard #: 22VAC40-665-580-B
Description: Based on staff interviews and records reviewed, the vendor did not ensure within the first 90 days of employment all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff.

Evidence: The records for staff #1, date of hire 11-9-2020, did not include documentation of obtaining the VA Preservice Training within 90 days of employment.

Plan of Correction: Va preservice training will be completed within 90 days of employment for future employment.
