An unannounced code compliance inspection was conducted on 6/14/2023 from 9:00 AM to 10:15 AM. There were 29 children present, ranging in age from two to five years, with 8 supervising staff. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, and procedures. A total of 5 children?s records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard #: 22.1-289.035-B-4 Description: Based on records review and staff interview, the facility did not obtain results of an out-of-state central registry and criminal history search for an employee who has resided outside of Virginia within the last 5 years.
Evidence:
1. Staff # 3's (DOH: 8/2021) sworn statement form dated 6/16/2022 indicated that they resided outside of Virginia within the last 5 years.
2. There was no documentation of the out-of-state criminal record check and central registry results for Staff #3 from a state in which they had listed as residing within the previous five years.Plan of Correction: I will submit the clearance for the out-of-state background checks by 6/16/2023.
