Comments:
An unannounced Monitoring Inspection was conducted today from 3:30pm to 4:30pm. There were 25 school-agers directly supervised by 2 staff. The physical plant, 5 staff records, 5 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, arriving to the cafeteria from the gym. Preparing for snack. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. If you have any questions regarding this inspection, please contact the Licensing Inspector. Standard #: 22VAC40-185-60-A
Description: Based on review, the facility did not ensure that children's records were maintained with the required information.
Evidence: Reviewed 5 children's records and found that:
1) Child 3 did not have completed addresses for either emergency contact.
2) Child 5 did not have a completed address for their second emergency contact.
3) Child 4 did not have a second emergency contact.
4) Child 5 did not have documentation of parent work telephone numbers.

Plan of Correction: Will obtain missing information.

Standard #: 22VAC40-185-560-F
Description: Based on review and observation, the facility did not ensure that the menu of the current one week period indicated substitutions.
Evidence: Reviewed the menu and found that it stated: salad, cheese and dressing. Instead the children were given crackers and carrots.

Plan of Correction: Will add any changes to the posted menu from now on.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the facility did not have documentation of their most recent violation notice posted.
Evidence: Observed that the most recent posted inspection notice was dated 10/30/18. The facility's last inspection was on 5/22/19.

Plan of Correction: Will post most today's inspection report.
