A report of an injury that required outside medical treatment was self-reported by the center on 6/20/2022. The inspector reviewed documentation related to the incident and conducted interviews. Information gathered during the inspection determined non-compliance with applicable standards of law and a violation was documented on the violation notice.

Standard #: 8VAC20-780-80-B-2 Description: Based on review of an injury report and interview, the center did not inform the department's representative as soon as practical, but not to exceed two business days, of any injury to a child that occurred while the child was under the supervision of the center and that required outside medical attention.
Evidence:
1. The center Director reported a serious injury to the department's representative on 6/20/2022.
2. The date of the injury was documented on the injury report as 6/14/2022.
3. Programs are required to contact the department of any injury that requires outside medical attention no more that two business days after the injury occurs.Plan of Correction: The Director reported the injury to the parents as soon as it happened based on center protocol and the safety of the child. Mom chose to pick up the child as a precautionary step to get checked by medical personnel. After child was checked to be healthy and no medical treatment required, child returned to care. Steps to follow protocol for the company as well as reporting to licensing agency were followed. A retraining with managers to follow the 2 day process was completed.
