An unannounced monitoring inspection was conducted today from 3:20pm-5:40pm. There were 36 children (infants - school-age) directly supervised by 7 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, coloring and doing their homework. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Standard #: 8VAC20-780-130-E Description: Based on record review, the center did not obtain documentation of additional Immunizations once every six months for children under the age of two years.
Evidence:
Child #4's record contains an immunization record that was last updated on 8/19/2022.Plan of Correction: The child had turned 1 within the past month. Records were updated by parent within the same week.
Last immunization 1/25/2023.
Standard #: 8VAC20-780-40-M Description: Based on observations and interview, a current written list of all children's allergies, sensitivities, and dietary restrictions was not kept in each room or area where children are present.
Evidence:
The written list of all children's allergies, sensitivities, and dietary restrictions was not available for review in the art room while children were using the classroom.Plan of Correction: Allergy list was added to art enrichment room.
Standard #: 8VAC20-780-70 Description: Based on record review, required records were not kept for each staff person.
Evidence:
Staff #3's record is missing two references as to their character and reputation, as well as competency required prior to the first day of employment.Plan of Correction: References were on site but mis-filed. They have been filed in the correct folder on site.
Standard #: 8VAC20-780-350-B-5 Description: Based on observation and staff interviews, ratios were not maintained in the school age classroom.
Evidence:
1. On the day of the inspection, 19 children were observed with Staff #1 when the inspector entered the school-age classroom at approximately 3:25 pm.
2. At approximately 3:30pm Staff #2 entered the classroom to assist Staff #1 in the School-age classroom.
3. The required ratio for the school-age classroom is 18 children to 1 staff person.Plan of Correction: Children were returning from bus runs and when the teacher returned, a teacher came into classroom to ensure ratio was covered.
Standard #: 8VAC20-780-560-F Description: Based on observation the center did not ensure that the current posted menu was dated for the current one-week period.
Evidence:
The menu posted was not dated with the most current dates, the posted menu was dated 2/27/2023-3/3/2023.Plan of Correction: Menu was current but the date was a mis-print.
Standard #: 8VAC20-820-120-E-2 Description: Based on record review, the center did not post the findings of the most recent inspection of the facility.

The inspection results posted are dated 4/25/2022. The most current inspection prior to today's inspection was dated 10/19/2022.Plan of Correction: Inspection report was sent from the Licensing Inspector and was posted.
