An unannounced monitoring inspection was conducted 4/22/2024 from approximately 10:18am-12:00pm. Three classrooms were observed with total of 22 children directly supervised by 4 staff, all supervision ratios were in compliance. The physical plant, 2 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed playing outside out equipment, doing a table activity with Playdough, napping and listening to/singing along with music. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

Evidence: Red tube of playground equipment had two screws that were exposed and allowed enough space for fingers to be pinched or snag skin.Plan of Correction: Director stated they would dispose of equipment.
