Technical Assistance:
Discussed ways to organize the completed annual training documentation. A model form to document annual staff training will be sent to the provider.

Comments:
An unannounced Monitoring inspection was conducted today. There were a total of 9 children
(27 pts) with 2 staff providing supervision today. At the beginning of the inspection the children were finishing lunch. The children were observed eating lunch and drinking milk or water. After lunch, staff took each child to the bathroom, or diapered and washed hands. The children prepared to transition to nap time. Staff were observed putting sleep sacks on some of the children before placing them on their backs in cribs. Observation of the programming space which included the play area, cot room, crib room, bathroom and kitchen area was made. The home was maintained and organized with toys, equipment and books that were determined age and stage appropriate for the children currently in care. No medication needs or food allergies identified at this time. Early programming consisted of making painted strawberry handprints which were drying in the children's cubby areas. All children's files (9), all staff (2) and 1 household file were reviewed. The previous violations from the last inspection were reviewed and one repeated violation found along with other identified non-compliances. This inspection occurred between 11am and 2pm. Based on child record review, verification that the following information was provided to parents was not found.
Evidence:
Documentation was not found that Child # 2's parents were provided with the provisions of the emergency preparedness plan and whether the provider has liability insurance.
