Comments:
A monitoring inspection was initiated on 12/9/2022 and concluded on 12/9/2022. There were 32 children present with 12 staff supervising, one office staff and one cook present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and one staff records were reviewed. The inspection started at 1:00 pm and concluded at 2:30 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-240-B
Description: Based on record reviews the center failed to ensure that staff shall complete orientation training prior to the staff working alone with children and no later than seven days of assuming job responsibilities.
Evidence:
Staff #1, 2, 3 did not have documentation of orientation training being completed prior to working alone with children.

Plan of Correction: Ensure documentation of training is included in staff files as they are completed before 1st day of work [sic]

Standard #: 8VAC20-780-280-B
Description: Based on observation the center failed to ensure that hazardous shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. Located on the sink in the infant area was Clorox wipes, disinfectant spray not locked.
2. Located on a shelf next to the refrigerator in the infant area was glass cleaner, not locked.

Plan of Correction: Ensure staff locks hazardous items every time. [sic]

Standard #: 8VAC20-780-340-F
Description: Based on observation the center failed to ensure that children under 10 years of age were always within actual sight and sound supervision of staff.
Evidence:
In the pre-k area during nap time two children were placed behind shelves to sleep. The six staff in this area remained at a table eating lunch approximately 25 feet from the children. The staff could not see the two children during nap time.

Plan of Correction: Ensure staff keeps sight and sound supervision of ALL children even during nap. [sic]
