The next anticipated inspection will be the providers renewal inspection. Provider was reminded to have all renewal application documents submitted through the provider portal 45 days prior to expiration for processing. Links to the portal can be found on the VDOE website.

An unannounced monitoring inspection was conducted today between 9:25am and 11:20am. There were 7 children present with the Provider and 1 assistant for a total of 16 points within the supervision guidelines. The children were observed doing exercises, painting, playing outside and other organized activities. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard #: 8VAC20-800-750-D Description: Based on observation, over the counter topical ointments were used beyond the expiration date of the product.
Evidence: Child #3's Aquaphor expired 11/2021.Plan of Correction: I will throw it out or give to the parents.
Standard #: 8VAC20-800-60-B Description: Based on record review, children's records did not contain all required information.
Evidence: Child #2 and Child #3's records did not contain addresses for their emergency contacts.Plan of Correction: I'll ask the parents today to fill the addresses in.
Standard #: 8VAC20-800-90-C Description: Based on record review, the family day home did not obtain documentation of additional immunizations every six months for each child under the age of two years.
Evidence: Child #2 (21 months old) most recent immunizations are from when the child was 6 months old.Plan of Correction: I'll ask the parents today to bring the records in.
Standard #: 8VAC20-800-460-C Description: Based on record review, a dog was not vaccinated for rabies.
Evidence: The Provider's dog's most recent vet records showed that the dogs rabies vaccination expired 1/31/2023.Plan of Correction: I'll call the vet today to make an appointment.
Standard #: 8VAC20-800-500-E Description: Based on observation, sandboxes were not covered when not in use.
Evidence: There was a large sandbox area of the providers backyard. The sandbox was not covered.Plan of Correction: We did cover it but the wind must have blown it off because we didn't come outside yesterday to see it blew off.
Standard #: 8VAC20-820-120-E-6 Description: Based on observation and interview, a copy of a special order issued by the department was not posted in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.
Evidence: The licensees special order advising consumers of serious and persistent violations was not posted at the public entrance of the family day home. The Provider confirmed that they have not posted the special order.Plan of Correction: I'll print it and post it to the door.
