An unannounced monitoring inspection was conducted today between 9:45am and 10:50am. There were 7 children present with the Provider for a total of 18 points outside the supervision guidelines. The children were observed participating in circle time, practicing shapes with flash cards and playing with play doh. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Standard #: 8VAC20-800-940-A Description: Repeat violation.
Based on observation, refrigerator temperatures were not maintained at or below 40 degrees.
Evidence: The thermostat on the refrigerator was 42 degrees during the inspection.Plan of Correction: My sister-in-law was cleaning out the refrigerator and it must have made the temperature go up. I will check the temperature.
Standard #: 8VAC20-800-50-A Description: Repeat violation.
Based on record review, the family day home does not keep an accurate record of children in attendance each day.
Evidence: There were 6 children's names documented on the providers daily attendance, there were 7 children present during the inspection. The 6 children's names did not match the names of the children present.Plan of Correction: I filled it out quickly and must have wrote down the wrong name.
Standard #: 8VAC20-800-180-A Description: Repeat violation.
Based on record review, the provider did not obtain a subsequent tuberculosis (TB) screening every two years.
Evidence: The providers most recent TB screening is dated 9/1/2021.Plan of Correction: I will make an appointment to go before Monday.
Standard #: 8VAC20-800-570-A Description: Based on observation, the provider did not ensure to not exceed 16 points.
Evidence: The provider was observed supervising two infants (8 points) and five two year old through four year old children (10 points), for a total of 18 points.Plan of Correction: One of the children is not usually here, but they are here today. I am working to try and hire someone new.
