Comments:
A monitoring inspection was initiated on 10/24/2023 and concluded on 10/24/2023. There were nine children present with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and three staff records were reviewed. The inspection started at 11:55 am and concluded at 1:30 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and date to be corrected? for each violation cited on the violation notice and return it to me within five (5) business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring gain; and 3) person or person(s) responsible for implementation.

Standard #: 22.1-289.011-F
Description: Based on observation the center failed to ensure the license and any other documents required by the Superintendent shall be posted in a conspicuous place on the licensed premises.
Evidence:
The violation notice from 4/11/23 was not posted in a conspicuous place.

Plan of Correction: Placed immediately

Standard #: 22.1-289.035-B-2
Description: Based on record reviews the center failed to ensure that staff submit to a fingerprint based background check prior to the first day of employment.
Evidence:
Staff #3 (hire date 8/21/23) does not have a fingerprint background check on file.

Plan of Correction: Background check will be done before hired

Standard #: 8VAC20-770-60-C-2
Description: Based on record reviews the center failed to ensure that staff have a central registry finding within 30 days of employment.
Evidence:
Staff #2 (hire date 8/31/23) does not have a central registry check on file.

Plan of Correction: Central registry was sent 10/19/23 waiting for it to come back

Standard #: 8VAC20-780-210-A
Description: REPEAT VIOLATION
Based on interviews the center failed to ensure that program leaders meet the educational requirements.
Evidence:
Staff #2 was in a program/lead teacher position on 10/25/23. Staff #2 does not meet the qualifications to be a lead teacher.

Plan of Correction: Staff will be lead teacher on 11/21/23. Staff will be observed till then.

Standard #: 8VAC20-780-280-B
Description: Based on observation the center failed to ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence:
Located in the white cabinet labeled staff snacks was a can of disinfectant spray not locked. This cabinet is accessible to the pre-school/school age classroom.
Located in the staff bathroom was a spray can of deodorizer, not locked. This area is accessible to the pre-school/school age classroom

Plan of Correction: Substances were removed and put in locked cabinet

Standard #: 8VAC20-780-500-A
Description: Based on interviews the center failed to ensure that children's hands are washed after meals and snacks.
Evidence:
While interviewing staff it was found that handwashing after meals was not being done.

Plan of Correction: Make sure all staff know to handwash
