An unannounced complaint inspection was completed from 10:05 until 11:35 based on a complaint received in this office 10/24/23. Interviews were held with staff.

Standard #: 8VAC20-780-340-F Complaint related: Yes Description: Based on interviews, the center did not always maintain sight and sound supervision of children under 10 years of age.
Evidence:
1. Witness #1 stated that on 10/24/23, a preschool age child was observed alone on the playground at approximately 2:00pm. The witness then took the child back to her classroom. Staff #1 verified that child #1 was left alone on the playground for approximately 1-2 minutes.Plan of Correction: Children will be kept in sight and sound supervision at all times and no be left unsupervised on the playground.
