Comments:
A renewal inspection was conducted on 09/06/2023 from approximately 10:17 a.m. ? 3:05 p.m. There were 56 children present ranging from infants to five years old and twelve staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Five child records, five staff records and four Board Members were reviewed. The information gathered during the inspection determined violations with applicable standards or law. A violation notice was issued. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 10/02/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on record review and interview, the provider did not ensure that non-prescription medication was only administered to a child with written authorization from the parent.
Evidence:
During an interview with staff, the staff described administering teething gel to an infant that was provided by the child?s parent for teething pain that morning. Licensing inspector reviewed the child?s record and did not observe a written authorization from this child?s parent to administer this nonprescription medication as required by 8VAC20-780-510.B.2

Description: Based on observation, the center did not ensure that medication shall be labeled with the child?s name, the name of the medication, the dosage amount, and the time or times to be given.
Evidence:
A physician prescribed medication identified by center staff for C5 did not contain the pharmacy label that included the child?s name, the name of the medication, the dosage amount, and the time or times to be given.
