Comments:
This inspection was conducted by licensing staff using an alternative remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A complaint inspection was initiated on 02/08/2021 and concluded on 02/12/2021. A complaint incident was received by the department regarding allegations in the areas of Provider Homes and Children Services. The Quality Manager was contacted by telephone and email to conduct the investigation.
The inspector emailed the Quality Manager a list of documentation required to complete the investigation. The inspector reviewed 1 child record and 1 provider home record submitted by the agency to ensure documentation was complete.
The evidence gathered during the investigation determined a valid complaint and supported the allegations of non-compliance with standards or law, and a violation was issued. Any violations not related to the complaint by identified during the course of the investigation can be found on the violation notice. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public website within five business days, regardless of whether or not the Plan of Correction is completed.

Description: Based on the review of the file for Foster Home (FH)-1 and based on an interview with agency representatives (AR), the licensee failed to document other correspondence or information pertaining to the home including a narrative of any concerns the licensee has about the status of the home.
Findings:
1. Based on the foster home records presented for FH-1, the licensee failed to document information pertaining to the home including a narrative of concerns related to an incident on 07/25/2020.
2. During an exit interview with AR-1 and AR-3, reviewed the record and were not able to locate documentation of correspondents or information or narrative addressing concerns related to the 07/25/2020 incident.
