Technical Assistance provided regarding standard 22VAC40-131-230.E.6, considering all new information, specifically addressing safety concerns surrounding changes in driving records of foster home providers during the certification period.

This inspection was conducted by licensing staff using an alternative remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A monitoring inspection was initiated on 04/12/2021 and concluded on 04/21/2021. The Quality Assurance Manager was contacted by email to initiate the inspection. There were 41 children in care, and 32 approved provider homes. The agency reports no newly hired staff since the last inspection. The inspector emailed the Quality Assurance Manager a list of items required to complete the inspection. The inspector reviewed 4 children?s records, 3 provider home records and board officer records submitted by the agency to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the agency. Any violations can be found on the violation notice. The licensee has five business days from receipt of the inspection documents to complete the section entitled "Plan of correction," sign each page of the inspection documents and return to the Licensing Office. The licensee should retain a copy to be posted at the facility. Results of the inspection are subject to public disclosure and will be posted on the Virginia Department of Social Services public website within five business days, regardless of whether or not the Plan of Correction is completed.

Standard #: 22VAC40-131-250-L Description: Based on the review of the foster child (FC-3) record and based on an interview with agency representatives (AR), the licensee failed to document an interview with the child prior to the child?s placement.
Findings
1.Based on the review of the records presented, there was no evidence of an interview with FC-3 child prior to the child?s placement.
2.During an exit interview on 04/21/2021, AR-1 acknowledged the absence of evidence of an interview with FC-3 prior to placement.Plan of Correction: The narrative for the interview with child prior to the pre-placement visit was missing from the file. To prevent this oversight, documentation of interview will also be included on the IPOC.
