Technical Assistance:
Technical Assistance was provided in the following areas:
22VAC40-131-160-Discussed maintaining documentation of details substantiating qualifications of employees and discussed maintaining references for employees, volunteers and interns.
22VAC40-131-340.F-engagement of birth parents in the development of individualized service plans, treatment plans and quarterly progress reports; discussed avenues for increase documentation of birth parent involvement.

Comments:
A monitoring inspection was initiated on October 28, 2021 and concluded on November 4, 2021. The Quality Assurance Manager was contacted by telephone and email to initiate the inspection. The Quality Assurance Manager reported the current census was thirty-two (32) active child placements, thirty-seven (37) active provider homes and five (5) newly hired staff since the last inspection. The Inspector emailed the Quality Assurance Manager a list of items require to complete the remote documentation review portion of the inspection. The inspector reviewed three (3) active child placement records, four (4) complete provider home records (including one re-evaluated home record), one (1) additional provider home record (background checks only) and six (6) personnel records. The inspector review updated policies and procedures as well as staffing information. An exit interview was conducted with agency representatives on the date of the inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violation were documented on the violation notice issued to the facility.

Standard #: 22VAC40-131-290-E
Description: Based on the review of the records provided for Foster Child-3 (FC-3) and based on an interview with agency representatives, the licensee failed to file a copy of a dental examination report in the child?s record which contained the signature of a licensed dentist or his designee.

Findings:
1.Based on the review of the records, there was no dental examination report presented for review.
2.During the exit interview, AR1 shared a dental history report summarizing all the dental encounters that FC-3 had with his dentist over the last year however, no dental examination report which contained the signature of a licensed dentist or designee was shared.
3.During the exit interview, AR-1 and AR-2 reviewed the record. During this interview, AR-1 acknowledged the absence of a copy of a dental examination report in the child?s record which contained the signature of a licensed dentist or his designee.
