22VAC-131-3250F. The Licensee shall ensure that shall in addition to the requirements specified in subsection B of this section, evaluate and update the comprehensive treatment and service plan for the upcoming year.

Comments:
Type of inspection: Renewal

Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 4/17/2023 from 11:30 a.m. to 4:00p.m. and on 4/18/2023 from 9:30 a.m. to 2:00p.m.

The Licensing Inspector met with the Executive Director and the CFO upon arriving to the location and to discuss the entrance to the inspection. There was one new staff member at the location since the last inspection and the licensing inspector interviewed this staff member.

Observations by licensing inspector: posting of the license and inspection documents was observed.
There were no major changes made to the policies or procedures since the last inspection. The Licensing Inspector inquired about annual reports to assess what was working well and areas of improvement.

The Licensing Inspector interviewed the (1) Family Counselor regarding job duties and responsibilities as interns, training, and supervision.

The evidence gathered during the inspection determined (1) violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website within five (5) business days of your receipt of the inspection summary. Description: Violation:
Based on the record review, the agency failed to ensure that the home visit narratives completed for FC(2) were up to date and that the narrative sections were original information and not completed as duplicates.
1) (FC-2) was admitted on 7/6/2022.
2) The case narrative reviews contained duplicate sections for the 10/5/2022 and 10/27/2022 home visits.
3) The findings were discussed during the preliminary findings review and the staff acknowledged the findings.
4) The findings were reviewed during the exit interview.
