Comments:

An unannounced monitoring inspection was conducted on 4/21/2022. At the time of the inspection there were 17 residents in care. Three resident and three staff files were reviewed as well as other required documentation. There were no staff hired since the last inspection. Menus, activity schedules and all required postings were present. Health care oversight, medication administration, audit of the medication cart, fire drills and emergency preparedness plans were reviewed etc. Building and grounds were inspected. An exit meeting was held with the Administrator. Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today.

Description:	Based on review of resident records the facility failed to retain written acknowledgment of the receipt of the disclosure of information about the facility. Evidence: Written acknowledgment of receipt of disclosure of information about the facility was not present in the file for resident # 2.

Standard #:	22VAC40-73-720-A
Description:	Based on review of resident records the facility failed to ensure that written DNR orders are included in the resident individualized service plan. Evidence: Resident #1 was admitted on 10/19/07 and has a valid DNR order present in his file. However, there is no DNR goal on his individualized service plan dated 4/16/21.
