Comments:
An unannounced renewal inspection was conducted on 1/6/23 (7:40 AM - 11:25 AM). At the time of entrance, seven residents were in care. Meals, medication administration, and an activity were observed. Building and grounds were inspected and records were reviewed. The sample size consisted of four resident records and three staff records. Violation was discussed and an exit meeting was held.

The evidence gathered during the inspection determined non-compliance with applicable standard(s) or law, and violation(s) were documented on the violation notice issued to the facility. The licensee has the opportunity to submit a plan of correction to indicate how the cited violation(s) will be addressed in order to return the facility to compliance and maintain future compliance with applicable standard(s) or law.

If the licensee wishes to provide a plan of correction: (i) type the plan on a separate Word document, (ii) identify the standard violation number being addressed, (iii) include the date the violation will be corrected, (IV) do not include any names or confidential information, and (V) return to the licensing inspector by email within five (5) business days of the exit interview.

Violations:
Standard #: 22VAC40-73-520-I
Description: Based on observation, the facility failed to ensure that the written schedule of activities is updated. If one activity is substituted for another, the change shall be noted on the schedule.
Evidence: Residents were observed playing bingo at 9:45 AM and watching television at 11:10 AM. Exercise/Dancing was listed as the activity for 11:00 AM. The activity schedule was not updated to reflect the changes that were made.

Plan of Correction: Administrator will retrain herself and staff to document any changes and substitutions made to the activity calendar. The residents at the home had asked to play bingo and listen to music for the morning activity, hence the change.
