Central Registry Form

Users may print and complete the Central Registry Search form (English). Please follow the instructions carefully so your form can be processed without any delays.

Facility Search

The Office of Background Investigations (OBI) has updated the way providers search for their facility name as part of our work to improve portal registration.

Providers who register for access to the OBI Provider Portal and receive criminal background fingerprint results from VDSS OBI can now search for their facility using only part of the facility name. You no longer need to enter the full facility name, but you must still enter the city, state, and ZIP code.
