An unannounced monitoring inspection was conducted on 2/28/19. At the time of entrance 19participants were present with nine center staff. This exceeds the required ratio of one staff per six participants. The sample size consisted of five participant records and six staff records including the criminal background checks on all new staff hired since previous inspection. Participants were observed eating and engaging in activities including music therapy, fitness group, art, and daily chronicle. Medications were reviewed.
