Comments:

An unannounced mandated monitoring inspection was conducted 5/13/2019. At the time of entrance there were approximately 36 participants in care and six staff members providing supervision. Four participant and four staff records were reviewed. No participants received medications during the inspection. Activities observed included current events, traditional games, karaoke and morning exercise. Criminal Background Checks of all staff hired since previous inspection conducted on 1/16/2019 were reviewed. Space was re-measured to assess increase in capacity. Participant interviews were not conducted because the participants? first language is not English and no interpreters were provided. Violation notice issued with the assessed risk reviewed and exit interview held. Standard #:	22VAC40-60-600-A
Description:	Center failed to ensure that the participant's screening for tuberculosis and a physical examination by or under the direction of a licensed physician shall be obtained either within 30 days prior to acceptance for admission or within 30 days prior to admission. Evidence: Participant #1 was admitted on 4/1/19 the tuberculosis and a physical examination are dated 7/25/18, Participant #2 was admitted 2/4/19 the tuberculosis exam is dated 3/25/16 and Participant #3 was admitted 2/4/19 the tuberculosis exam is dated 4/19/17.
Plan of Correction:	Center Director will review every participant's file to ensure the tuberculosis and a physical examination is obtained either within 30 days prior to acceptance for admission or within 30 days prior to admission.

Description:	Center failed to ensure that the record of the required evacuation drills documented the time required to evacuate and problems encountered, if any.
