Comments:
Two licensing inspectors conducted an unannounced mandated monitoring inspection on 05/16/2019; starting at 10:30 am and concluding at 12:45 pm. The center reported there are 55 participants enrolled with 20 participants attending and 6 staff providing care and services on the day of the inspection. A sample of participant and staff records were reviewed, the building was observed, lunch was observed, and interaction between staff and participants was observed. An exit meeting was conducted with the center director's designee on 05/20/2019 and at that time opportunity was given to find items not available in files. The findings of the inspection were reviewed. As a result of this inspection 5 violations are being cited. Please complete the "description of action to be taken" and "date to be corrected" for each violation cited on the violation notice, and then return the signed and dated copy to the licensing office within 10 calendar days (05/30/2019) of receipt. Violations:
Standard #: 22VAC40-60-235-C-2-d
Description: Based on documentation review, the facility failed to have subsequent evaluations for tuberculosis annually by a physician or an official of the local health department on one staff person in the sample of three. EVIDENCE: 1. Staff # 2 was hired on 03/31/2017. 2. The last tuberculosis evaluation completed by a physician or an official of the local health department was dated 04/04/2018 for staff # 2.

Plan of Correction: Working with HR to establish a spread sheet to reveal hire dates to ensure accurate tracking. [sic]

Standard #: 22VAC40-60-460-A
Description: 460A Based on observations made during the morning tour of the building, the facility failed to ensure that all areas of the facility were well lit for the safety and comfort of the participants. EVIDENCE: 1. There were two overhead fluorescent lights that were inoperable in the activities room.

Plan of Correction: Lighting problem had been reported to maintenance earlier on AM of 05/20/19. Light bulbs replaced late afternoon 5/20/19. [sic]

Standard #: 22VAC40-60-570-C
Description: Based on review of participant records, the facility failed to ensure the written plan of care be reviewed and updated at least every six months for one participant in a sample of six. EVIDENCE: 1. Participant # 3 had a plan of care completed in June of 2018 and again in February of 2019. This was an eight month time period between the update of the plan of care.

Plan of Correction: Construct spread sheet while conducting annual assessments to ensure that 6 mo. updates are completed in a timely way. [sic]

Standard #: 22VAC40-60-980-A
Description: Based on observations made during the tour of the building, the facility failed to include all necessary items in the first aid kit. EVIDENCE: 1. The first aid kit did not contain bee sting swabs or preparation.

Plan of Correction: Bee sting swabs were actually in First Aid Kit, but not clearly labeled. Contents were more clearly labeled inside kit. [sic]

Standard #: 22VAC40-60-980-B
Description: Based on observations made during the morning tour of the building, the facility failed to store the first aid kit so that is easily accessible to staff but not to participants. EVIDENCE: 1. Licensing Inspector observed the first aid kit to be unlocked on the bottom shelf of a 3 foot tall shelf sitting directly beside the reception desk at the entrance of the adult day care. This allows for easy access by the participants.

Plan of Correction: First Aid Kit is stored in office of Activity Director. [sic]
