Technical Assistance:

Technical assistance provided to the center's director regarding standard 554-B regarding sufficient storage space for equipment material and supplies used during the program.

Comments:

An unannounced renewal study was completed on May 08, 2019 from 2:05 p.m. to 4:26 p.m. For Central Adult Day Care Center. Upon arrival the licensing representative met with the center?s director. The center reported the census as twenty five participants in care. After, the entrance conference participants were observed in daily activities and the direct care staff members were observed providing direct care services. The record review encompassed nine (09) participant records and four (4) staff records. New violations were cited during the renewal study. As you fill in your portion of the violation notice, you will need to specify how the deficient practice will be or has been corrected. Writing the word ?corrected? is not acceptable under any circumstance. Your description of action to be taken must contain the following: 1) Steps to correct the noncompliance with the standard (s) 2) Measures to prevent the noncompliance from occurring again, and 3) Person (s) responsible for implementing each step and / or monitoring any preventive measure(s). This violation notice is subject to public disclosure and will be posted on the Virginia Department of Social Services? web site within 10 calendar days of your receipt of this letter, regardless of whether you return your description of action to be taken. Description:	VIOLATION: Based upon the record review, the facility failed to ensure that a sworn disclosure statement was signed and placed in each staff record. EVIDENCE: During the record review, staff record #1 failed to have a signed sworn disclosure in the record. The facility failed to provide evidence that would dispute the cited violation

Standard #:	22VAC40-60-580-A
Description:	VIOLATION: Based upon the record review and a staff interview , the facility failed to obtain a written participant agreement for each participant in care. EVIDENCE: During the renewal inspection, staff #2 confirmed that participant XYZ was included in the renewal census but the identified participant failed to have a written agreement on file. The record provided to the licensing representative for participant XYZ failed to have evidence that a written agreement was provided to the participant's or to the participant's legal representative in regards to the services provided by the center. The center failed to provide evidence that would dispute the cited violation.

Description:	VIOLATION: Based upon the record review, the center failed to ensure that each participant record contained a current photograph or written physical description of the participant. EVIDENCE: Participant record XYZ failed to have a photo of the identified participant or a physical description. The center failed to provide evidence that would dispute the cited violation.

Description:	VIOLATION: Based upon the record review, the center failed to provide evidence that a physical examination was provided to each participant in care prior to the participant's admission. EVIDENCE: During the record review, the record for participant XYZ failed to have any evidence that a physical examination was completed prior to the participant's admission or attendance. During the interview, staff #2 stated that participant XYZ did not have a physical examination because the participant was just visiting. The center failed to provide evidence that would dispute the cited violation.
