An unannounced mandated monitoring inspection was conducted on 5/30/19. At the time of entrance one participant was present, six participants are enrolled and three center staff were present. The sample size consisted of two participant records and two staff records. Participant and staff records and other documentation reviewed. No new staff have been hired since the previous inspection was conducted in November, 2017. Breakfast was observed and activities scheduled for the day include balloon volleyball, music and exercise. No participants received medications during the inspection. No violations cited today and exit interview held.
