Comments:

This was an unannounced monitoring inspection conducted by the Licensing Inspector from the Eastern Regional Office. The inspection was conducted on May 31, 2019 from 10:24 AM until 3:15 PM. There were 14 participants present. During the inspection several activities were observed to include Ball toss/exercise, ping-pong, word games, and puzzles. The lunch meal was also observed as posted on the menu to include Pork roast, beans, carrots, dinner roll and oranges. A tour of the center was conducted. Participant records and staff records were reviewed to include criminal background report for one new staff since the last inspection. Participant interviews were conducted. Discussed time frames for preliminary plan of care to be completed prior to participant start of program. No violations cited.
